A 63 Year Old Gentleman & Father of 5 Grown Sons From Vancouver Washington

A 63 year old gentleman & father of 5 grown sons from Vancouver Washington ended up here at Care Corps after traveling several states & a personal referral from a friend that was staying at a shelter in Lincoln Nebraska. He was told that Care Corps could provide him with the opportunities to work on his issues. The issues that he shared that were holding him back was severe pain in both legs due to a traumatic injury that occurred as a result of walking across a street and hit by an automobile a few years back. This accident shattered both legs requiring extensive surgeries. He started to receive disability benefits and medical coverage as a result. Due to his problems causing him to relocate time after time he no longer was receiving his benefits & lost his medical coverage. The main goal when first introduced to Case Management was to get his pain under control. I was able to make arrangements to get his meds paid for, insurance for one month on his car & gas to transport him to Omaha to pick up his meds. This was done with the collaborations with UYSS Inc., Salvation Army Church & Care Corps; allowing him to follow through with the contact to Social Security to see what we needed to do to get his benefits started again. We are also working on getting his birth certificate. With these issues on the path of resolution he will be able to establish and maintain permanent housing.

This gentleman is now about to leave Care Corps Homeless Services. He is armed now with his Social Security Disability & Retirement Insurance reinstated & back on track. He has already received two checks from the Social Security department that has provided him with the means to get his car fixed. He is doing very well with his pain management. His driverç—´ license has expired recently. Will be receiving his birth certificate in the mail any day now; when that arrives he can go to the court house and renew his drivers license. As a result of all the hard work he was able to accomplish with the assistance of Care Corps Homeless Services he will be leaving soon to permanent housing. I am extremely pleased to have been a part of this process.
